Job Description

Akido is seeking a dedicated Administrative Medical Assistant to join our remote Care Coordination team. In this vital role, you will be the backbone of our patient care operations, managing referrals, coordinating medications, scheduling appointments, and ensuring accurate patient records. Your efforts will directly impact our ability to provide seamless, timely healthcare to underserved communities.

Success in this position means efficiently navigating complex healthcare systems to connect patients with the services they need, ensuring no detail is missed and every patient receives exceptional support.

You will be instrumental in creating, submitting, and tracking specialty and Enhanced Care Management (ECM) referrals, ensuring all authorizations and documentation are in order. You'll also play a key role in processing medication refill requests, liaising with pharmacies to resolve coverage issues and patient access barriers, and scheduling appointments across various care settings.

Maintaining accurate patient charts in our EHR and case management platforms, preparing for patient visits, and supporting post-visit documentation are core responsibilities. You will also serve as a primary administrative contact for our field teams and clinical staff, participating in daily huddles and contributing to workflow improvements.

Key Responsibilities

  • Create, submit, and manage specialty referrals and Enhanced Care Management (ECM) referrals.
  • Track referral status, authorizations, and outcomes, proactively resolving delays.
  • Coordinate with providers, case managers, and field teams for continuity of care.
  • Process medication refill requests and route to providers for approval.
  • Communicate with pharmacies to resolve coverage issues, prior authorizations, and medication access barriers.
  • Track medication fulfillment and escalate urgent issues.
  • Schedule and coordinate patient appointments across primary care, specialty care, and diagnostics.
  • Communicate appointment details to patients and field teams.
  • Manage reschedules, cancellations, and urgent scheduling requests.
  • Create and maintain patient charts in EHR and case management platforms.
  • Ensure demographic, insurance, referral, and clinical documentation is accurate and complete.
  • Prepare charts prior to visits and support post-visit documentation.
  • Serve as an administrative point of contact for field teams and clinical staff.
  • Participate in daily huddles and care coordination meetings.
  • Identify process gaps and contribute to improving workflows for referrals, scheduling, and medication management.

Qualifications

  • Medical Assistant certificate and current BLS required.
  • 1+ year experience as a Medical Assistant, care coordinator, or healthcare administrative role.
  • Strong experience with EHR systems.
  • Strong experience with referrals management.
  • Strong experience with appointment scheduling.
  • Strong experience with medication refill workflows.
  • Highly organized and detail-oriented.
  • Ability to manage multiple patient cases simultaneously.
  • Comfortable working across multiple platforms (EHR, referral portals, health plan systems, case management tools).
  • Excellent written and verbal communication skills.
  • Compassionate and professional demeanor when working with underserved populations.
  • Reliable, punctual, collaborative, and self-directed in a remote environment.
  • Bilingual preferred (Spanish a plus).
